Advice please

I am a 64 year old male who works 12 hours a day, six days a week. I am pre-diabetic and 15kgs overweight. I am usually out for lunch with clients three days a week. I haven't exercised for 25 years but the doctors now say I have to lose weight otherwise I may need a new hip soon. Where do I start?
