Directions unclear

I would like to know if I hold each leg extension for 10 sec because the direction indicates + 10? I have also seen hold for 10 without the plus. I really dont know the difference,

Hi @Checbenj

You only do the hold on the last rep. So 14 leg extensions with no hold, then one 10-second hold in the lifted position. Then change sides and repeat with the other leg.
The plus sign I think is just to indicate you do both the reps and the hold before switching sides. (Otherwise, you would normally do all reps of the first exercise before moving on to the second exercise.)
